The study examines the effects of prolonged Hypothermic Oxygenated Machine Perfusion (HOPE) on liver transplantation logistics and outcomes. Conducted across 12 European centers, this multicenter observational cohort study included 93 liver transplant recipients who underwent prolonged HOPE for 4 hours or more. The main reason for prolongation was unforeseen logistics, such as delays in operating room availability. Despite extended perfusion times, the study found that prolonged HOPE resulted in low postoperative complication rates, low transaminase levels, and excellent patient and graft survival rates, comparable to traditional HOPE durations.
