This review article explores the impact of ischemia-reperfusion injury (IRI) on hepatocellular carcinoma (HCC) recurrence after liver transplantation and examines how machine perfusion (MP), specifically Hypothermic Oxygenated Perfusion (HOPE), mitigates these risks. IRI creates a favorable environment for tumor recurrence by promoting inflammation, mitochondrial damage, and endothelial dysfunction. HOPE, by reducing IRI and protecting graft function, shows potential in lowering HCC recurrence, even when using extended criteria donor (ECD) and DCD livers.
