This multicenter cohort study investigates outcomes of redo-liver transplantation (redo-LT) using grafts treated with hypothermic oxygenated machine perfusion (HOPE) across seven Italian centers. The study included adult recipients of second or third liver transplants between 2016 and 2023. HOPE allowed for the use of a high proportion of extended criteria donor (ECD) grafts-including old age, DCD, steatosis, and prolonged ischemia-with early allograft dysfunction, dialysis requirement, and major Clavien grade complications observed in 39%, 15%, and 52% of cases, respectively. Twelve-month patient and graft survival were 78.3% and 73.9%. Graft survival was not affected by donor ECD status. The preliminary data suggest HOPE can facilitate safe redo-transplantation, even when using ECD grafts, achieving outcomes comparable with benchmarks for this high-risk population.
