This poster examines the systematic application of hypothermic oxygenated perfusion (HOPE) in liver transplantation using marginal and previously rejected grafts from donation after circulatory death (DCD) and extended criteria donors (ECD) at a single Italian center over five years. The retrospective study compared post-operative results among patients transplanted with standard, DCD, and previously refused grafts, all of whom received HOPE prior to transplantation. Despite increased utilization of marginal donors and longer cold ischemia times, no significant differences were observed in early allograft dysfunction, major complications, biliary issues, or short-term graft and patient survival compared to standard donors. These findings suggest that HOPE can optimize outcomes and safely expand the donor pool in liver transplantation programs
