This large observational study evaluated 1,202 liver transplants using hypothermic oxygenated machine perfusion (HOPE) across 22 European centers from 2012 to 2021. It included both donation after brain death (DBD) and donation after circulatory death (DCD) livers of varying risk. The 1-, 3-, and 5-year death-censored graft survival rates were 95%, 92%, and 91% for DBD, and 92%, 87%, and 81% for DCD. Graft survival was consistent across risk groups, with low rates of primary non-function and ischemic cholangiopathy. HOPE is now at IDEAL-D stage 4, supporting its routine clinical use.
