This review article discusses the mechanisms of ischemia-reperfusion injury (IRI) in marginal liver grafts, particularly in the context of Hypothermic Oxygenated Machine Perfusion (HOPE). IRI is a major cause of liver transplant complications, especially in ECD and DCD grafts, which are more susceptible to injury. HOPE has shown to reduce IRI by improving mitochondrial function, decreasing oxidative stress, and modulating inflammatory responses. The article highlights how HOPE, by optimizing liver graft metabolism, provides better outcomes than SCS in marginal grafts.
