This article reviews the role of Machine Perfusion, particularly HOPE in modulating the immune response following liver transplantation. The study examines how ischemia-reperfusion injury (IRI) activates the innate immune system, leading to complications such as inflammation, graft rejection, and fibrosis. HOPE plays a significant role in reducing mitochondrial damage, minimizing the production of reactive oxygen species (ROS), and decreasing the activation of pro-inflammatory molecules, thereby mitigating IRI. The article also discusses the potential of machine perfusion as a delivery tool for therapeutic agents aimed at further reducing inflammation.
