This poster from Dora Gómez Pasantes and colleagues at A Coruña University Hospital describes their experience using hypothermic oxygenated perfusion (HOPE) via the VitaSmart with 31 cadaveric liver transplants from July 2022 to September 2023. The technique was evaluated for impact on post-transplant liver function, complications, and graft viability. The median perfusion and cold ischemia times were 128 and 295 minutes, respectively. In the cohort, most observed complications were minor (Clavien I), with a smaller proportion of major complications, particularly for biliary strictures and surgical reinterventions. Notably, no severe post-reperfusion syndrome cases occurred, and perioperative outcomes and graft function were described as favorable, underscoring the ease and safety of HOPE in clinical practice. This experience supports the growing evidence that HOPE can reduce ischemia-reperfusion injury, lower peak liver transaminases, and may decrease major post-transplant complications including biliary strictures and hepatocellular carcinoma recurrence.
