This study evaluates the impact of Hypothermic Oxygenated Machine Perfusion (HOPE) on extended criteria donation (ECD) liver transplantation outcomes from Donation After Brain Death (DBD) donors. Conducted as a randomized controlled trial (RCT) across multiple centers, the study compared HOPE to static cold storage (SCS). The primary endpoint was peak ALT levels (a marker of liver injury) within the first 7 days post-transplant. The study found that HOPE significantly reduced liver injury, postoperative complications, and hospital/ICU stays, offering a safer and more effective approach to ECD liver transplantation.
