Detailed analysis of DCD biliary outcomes in the multicenter randomized controlled trial (Bridge to HOPE) that evaluated hypothermic oxygenated perfusion (HOPE) versus static cold storage (SCS) for DCD and increased-risk DBD liver transplantation in the US. Among DCD liver transplant recipients, HOPE significantly reduced the incidence and severity of biliary complications and ischemic cholangiopathy compared to SCS, with fewer cases requiring intervention and lower graft failure rates. The trial demonstrates the benefit of HOPE in lowering post-transplant biliary complications, including re-transplantation due to severe IC, and supports its wider adoption for improving outcomes in DCD liver grafts.
