This trial randomized 170 liver transplant recipients across ten European transplant centers, with 85 receiving HOPE treatment and 85 undergoing conventional cold storage. The primary analysis found no significant difference in the proportion of patients experiencing at least one Clavien ?III complication (51.8% in HOPE vs. 54.1% in SCS, p = 0.76). However, post hoc analysis revealed a 3.7-fold reduction in severe liver-related Clavien ?IIIb complications per patient in the HOPE group, and no cases of graft failure due to liver-related complications, compared to a 7% graft failure rate in the control group. These results suggest HOPE’s potential in reducing severe post-transplant complications, warranting further validation in future trials.
